Information Systems Access Request

Concord University employees can seek access to Banner, Argos, and/or MachForm by requesting that their supervisor complete the Application Access Request Form on their behalf. Once the form is submitted by the employee's supervisor, the following approvals are required (sent via email):


  1. Confirmation of submission by the employee's supervisor
  2. Approval/denial to specific data-groups by data-group owners
  3. Final approval by the Office of Technology Services


The employee may elect to complete virtual training before, during, or after the approval steps above are completed. Training and assessments must be completed before accounts will be provisioned.
